Event Entertainment Service Agreement · 2026–2027
This Event Entertainment Service Agreement ("Agreement") is entered into on the date of submission between DJ KW Services Inc. ("DJ KW Services") and the purchaser identified below ("Client"). The Client engages DJ KW Services to provide professional entertainment services for the event described in this Agreement.
Agreed Total Fee: confirmed by DJ KW Services in your countersigned agreement, based on the hours of entertainment and start/end times above. Setup and breakdown time are not included in the contracted entertainment hours.
Overtime: Any entertainment requested beyond the agreed end time is subject to an additional hourly fee (stated in your countersigned agreement), subject to DJ KW Services' availability and venue approval.
Gratuity: DJ/MC gratuity is not included in the contracted fee.
The reservation deposit is required to secure the event date. The remaining balance must be paid no later than the conclusion of the event by cash, QuickPay/Zelle, or certified check.
Business or personal checks may be sent by USPS, FedEx, or UPS only when the event is 30 or more days away. Checks should be made payable to DJ KW Services Inc. and mailed to 381 W Asbury Drive, Round Lake, IL 60073. QuickPay/Zelle or PayPal payments may be sent using (312) 823-9454. PayPal payments are subject to a 3% fee.
